REQUESTING A COPY OF YOUR COMPANY’S OIL WELL™ REPORT
When your request for credit from an oil provider has been denied based upon an Oil Well™ report:
Your Oil Well™ report is available to you at no charge (no more than one time annually) if your business has been denied credit from an oil provider based upon an Oil Well™ report within the past 60 days. In order to receive the complementary copy of your business’ Oil Well™ report you must submit your request in writing, on your company’s letterhead, and include the following information:
- The name of the Oil Provide who denied your company credit based upon an oil Well™ report, including the date you were notified of such denial. Please include a copy of the correspondence from the oil provider notifying you that it has declined to extend your business credit.
- Provide your full business name, including any variations, and any or prior names under which your company has operated for the past 10 years.
- List your company's current physical address(es) and P.O. Box(es).
- List your company's prior physical address(es) and P.O. Box(es) for the past 10 years.
- The request must be signed by an owner or officer of your company.
If you are requesting an additional Oil Well™ report in any calendar year, you should follow the instructions below on how to purchase you business’ Oil Well™ report.
When you have not been denied credit from an oil provider based upon an oil Well™ report:
You may purchase a copy of your company’s Oil Well™ report for a $45 fee at any time. In order to purchase a copy of your business’ Oil Well™ report you must submit your request in writing, on your company’s letterhead, and include the following information:
- List your full business name, including any variations, and any or prior names under which your company has operated for the past 10 years. Note that you must pay an additional report fee of $45 for each business name that you request an Oil Well™ report for.
- List your company's current physical address(es) and P.O. Box(es).
- List your company's prior physical address(es) and P.O. Box(es) for the past 10 years.
- The request must be signed by an owner or officer of your company.
- Include a check or money order for the appropriate amount ($45 multiplied by the number of names that your company does business as if you are requesting an Oil Well™ for all names AND include 8.625% sales tax if your business’ principal place of business is located in NY) payable to Risk Assessment Data, LLC.
